THANK YOU for taking the time to fill out this *professional learning* evaluation form.  It is adapted from the Guskey PD evaluation framework to provide constructive feedback to the project team as they seek to continuously improve products and services to schools and programs.  Your individual responses will be kept strictly confidential within the ALD4ALL team and the Center for Positive Practices, the project's external evaluator.  Only anonymous and collective results may be shared with your school or stakeholders and the public.
